Fill Supplier qualification questionnaire form here 2. Signed suppliers declaration form 3. Sign the WHH Code of Conduct 4. Copy of the Contractor's License/Registration certificate with tax proofs 5. Financial Statements for the last 6 months. 6. Details of Previous Similar Projects, including references, contracts 7. Detailed and updated company profile 8. CV of the related Engineers 9. List of construction equipment/machineries/assets such as tipper trucks, water tankers, concrete mixers, vibrators, and steel tanks, including pictures for evaluation
