  • Expression of Interest are invited for Diagnostic Centers Conducting Basic Lab Tests for MDR-TB Patients Scope of Work The selected diagnostic centers will be required to: 1. Conduct basic laboratory tests for MDR-TB patients. 2. Provide timely and accurate laboratory test results to MDR-TB treatment centers. 3. Maintain accurate records of laboratory test results and patient information. 4. Report laboratory test results to the appropriate authorities as required. 5. Maintain and calibrate laboratory equipment and ensure that it is in good working condition. 6. Ensure the safety of laboratory staff and patients by adhering to proper laboratory safety protocols. Pregnancy test OTHERS ECG Location: Mogadishu, Kismayo, Baidoa, Baladweyn, and Dhusamareb According to the WHO estimates for TB in Somalia, the TB incidence is estimated at 250 per 100,000 while the incidence of RR/MDR-TB is estimated at 12 per 100,000 (Global TB report 2021). A nationwide drug-resistance survey of cases of pulmonary sputum smear-positive TB in Somalia was conducted from March 2010- October 2011 by WHO in collaboration with NTP. The survey revealed MDR-TB among 5.2% and 40.8% of patients with new and previously treated TB, respectively. These levels of drug resistance are among the highest ever documented in Africa and the Middle East. Thus, drug resistance presents a serious challenge for TB control in Somalia. The major effect of Covid-19 in Somalia was an immediate decline in TB case finding, increased primary loss to follow particularly for MDR-TB cases due to restriction in movement and patients' fear of getting covid-19 when they go to hospitals or TB centres for treatment. All of the above-mentioned challenges were primarily due to lack of decentralized DR-TB services, inadequate specimen transportation system and network and lack of patient support mechanisms etc, this has led to opening of new MDR-TB decentralized sites in all states to bring the services closer to communities located far from the 3 hospitals to improve accessibility to treatment as well as reduce primary loss to follow up. The decentralized MDR-TB management focuses on supporting ambulatory and community-based care for MDR-TB by increasing the numbers of MDR-TB units in the States Currently, DR TB is managed at the 3 hospitals including the main Mogadishu MDR-TB facility in Forlanini hospital, and there are newly opened decentralized sites in Kismayo, Baidoa, Baladweyn, Dhusamareb, and others. DR-TB patients need baseline tests as part of initial diagnosis and for monitoring of treatment response as well as treatment side effects and all facilities don-t have all capabilities of conducting these tests specially those of newly opened in states, so there is need of contacting external public or private facility that can do these tests for patients to reduce catastrophic costs.
